Dear Provider,

You have been selected to participate in a Gestational Diabetes Mellitus and Postpartum Practices of APRNs in Kentucky Survey related to knowledge, attitudes, and practice concerning postpartum follow up for women previously diagnosed with gestational diabetes. Western Kentucky University’s Doctoral Student, Diana Wooden, MSN, RN, primary investigator, is conducting the study.

In attempt to develop a plan to prevent and detect type 2 diabetes among women with a history of gestational diabetes mellitus, feedback from midwives and advanced practice registered nurses is needed regarding your expertise in caring for women.

This survey is voluntary and confidential. The web-based questionnaire uses a secure socket layer (SSL) protocol that provides online security and your information will be encrypted. It should take you approximately 10-15 minutes to complete. Your participation will provide vital feedback for  diabetes prevention efforts and allow providers and public health to better support caring for women in Alaska. The risk to participating in this project is the inconvenience of taking the time to answer the questions. There are no financial costs to you associated with participation in this study. The benefits of the study are the chance to improve the health and health education of women.

To partially compensate you for your time, each participant has the opportunity to be entered into a drawing for a $25 gift card. You may enter the drawing by completing the incentive survey once you have competed the research study.
