Thank you for your time. The State of Alaska's Department of Health and Social Services, Section of Chronic Disease Prevention and Health Promotion needs your feedback.

The Section recently created a number of educational resources for patients.The "Get Screened. Take charge of your health. Take charge of your life." campaign and related materials are intended to encourage patients to get important wellness exams and preventive screenings.

The Section is hoping to learn more about the best ways to get this information to your patients, how to make this process easy and efficient for providers, and how to best communicate with you.
